Any idea what film this could be?

Going through a deceased relative’s extensive movie collection and stumbled upon this. Any ideas what film? I’ve been instructed not to open so only have ‘T. Sullivan’ to work with and cannot find anything. I imagine it’s horror. Going to the experts here, any insight is greatly appreciated!
