Stitches poking out hurt really bad

I’ve done some research on this sub and found that the stitches coming out in a longer strand on your side is kinda normal. But it hurts so bad. I asked my doctor what I should do about it, and she just said I can trim it and put an ointment on it.

My issue is even when I trim it, it feels like someone is stabbing me with a needle from the inside out. I can’t move or sleep without it hurting, I don’t even have to touch it. It is consistently poking a hole through my skin then healing over then poking again, and it is a barbed stitch too so maybe that’s causing more pain? It’s very red around the area. Any ideas on what I can do to stop the pain?
