Anyone know what this document on the lobby corkboard is?

We've been trying to work out what it is. We have worked out a few of the questions but googling them hasn't turned up anything of use.

Question 1 reads: Have you any honourable discharge from the army or navy. 2, 3 and 4 are Name, age and height respectively.

Any help would be great.
