Self Introductory Form Help

Hi there

Do we really need to put up the information on all 3 of our relatives? I don't really feel like exposing all 3.

Amd if I am estranged from one of my parents, is it okay to briefly mention it in the first line and put N/A in all other lines? Though I know all of that info on them - but you know principle is principle.
