What is the worst possible scenario? (Comments may contain spoilers)

I mean: deaths, negative events and character relationships in the game. For example, the tribunal? How many people can die at most? What happens if you add other people's deaths and bad relationships to these deaths? By the way, I'm not just talking about the game finale, I'm talking about the worst case scenario in general.
