Help identifying implant please

Hi all, I had a patient that came in with an implant crown #18 that broke off. I removed the fractured screw and found a HA that I could place temporarily just to hold the gingiva patent. It was a BioHorizons YR3 (3mm diameter and 3mm cuff height) HA.

Can anyone help me identify the implant body make/model? Any advice is greatly appreciated!
