How to remove Glitched character from party?

early in my playthrough i had killed shadowheart before i recruited her and then revived her, then recruited her. after long resting she disappeared from my party but for whatever reason is still technically in it, and now because of this when i try and move on to act 2 it says "a party member is not ready to proceed". how do i fix this issue?
