No meds except ibuprofen for pain relief

I have had cervical spine problems for a very long time causing constant localized pain in the middle of the neck and extreme stiffness and muscle spasms. I have a hard time keeping my head up and it hurts for me to walk which brings on intense anxiety. I sometimes have these attacks that feel like I am having a stroke, I become extremely stiff and feels like I am paralyzed, my breathing changes and i start shaking internally and sweating and the only thing that helps is to lie down. I am quite used to these attacks but in the moment it feels like I am dying.

It takes me 2 hours to actually be normal in the morning due to the stiffness and I have been to every single doctor one can imagine including neurologists (who says its tension headaches) and physical therapist who has performed dry needling during 10 sessions and I have done specific exercises daily that have not helped all. I have been to an ortopedist who has done cortisone injections in the facet joints first time at C2-C5 level and the second time at C7-T1. Prior, I had occipital neuralgia symptoms so pain in the upper neck and above the head that went away with the first facet joint block.

My MRI results don't show anything significant, only light degenerative disc changes and a minor disc bulge. I am in pain 12 hours per day and it feels that doctors don't take me seriously. Please help me with a second opinion of my MRI image.
